Also print out the form called Authorization of Mail Delivery through Agent. This authorizes the post office to release your mail to Mail Service Center.
fill out this form along with a copy of two forms of identification and mail to us at the address above: Also print out and sign this agreement to rent a mailbox" from the State of California. The Form 1583 must be notarized if you do not open your box in person.
(Note: No refunds given on box rental if you cancel your account before term is over)
Also note that the USPS does not honor forwarding requests for customers when they cancel service with us. We are a Commercial Mail Receiving Agency. When you cancel service, you must inform your correspondents of your new address or request that Mail Service Center forward your mail to you at regular forwarding charges.
